- comments: observed stoppers in three-compartment sink not able to close. Instructed person in charge to repair and maintain.

- comments: observed peeling paint and wall damage in the following areas: front brown wall near customer restroom, white wall leading to front cashier area, wall near door in first storage room off hallway, wall behind door to enter employee area. Instructed person in charge to repair and maintain.

- comments: observed excessive unused equipment throughout storage rooms in the facility. Observed items stored on the floor in storage rooms. Instructed the person in charge to remove unused equipment and maintain all items at least six inches from the floor at all times.
