Approximately 10 live small flying insects landed on the walls over the bread and clean plates at the front counter in the dining room. Approximately 10 live small flying insects landed on cases of single service items on shelves in the kitchen. Approximately 5 live small flying insects landed on shelving for single service items and clean cups in the kitchen. 1 live small flying insect landed on a cloth used to cover cold salads and sauces on the wait station in the kitchen. 5 live small flying insects landed on boxes of cooking oil on a shelf across from the cook line in the kitchen. Approximately 20 live small flying insects crawling on the floor and wall behind the stand mixer in the kitchen. Approximately 20 live small flying insects landed on bins of flour and rice in the kitchen. Priority: High Priority
- High Priority - Observed: Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At the wait station, butter packets (75F - Cold Holding). The item was held on the station at room temperature for less than 4 hours. The item was returned to refrigeration to chill and cold hold. **Corrective Action Taken** Priority: High Priority
