I ate at the Hazlet Costco on July 27, around noon. I had their hotdog(relish, and ketchup), and churro. Those two items where the only things I had all day. Later that night around 7, I had diarrhea and my stomach felt upset.  | Symptoms: Diarrhea

I bought the bag of frozen mixed berries last night. I opened them this morning and put them into a smoothie. I was feeling fine until about 30 min after eating the smoothie. About 2 hours after eating it, I threw it up.  | Symptoms: Vomiting
