- comments: observed no valid city of chicago foodservice sanitation certificate on site. Instructed person in charge that valid original city of chicago sanitation certificate must be on site at all times (photocopies and pictures not allowed). Priority foundation #7-38-012. Citation issued.

- comments: observed interior of ice machine with some build-up, in need of cleaning. Instructed person in charge to clean and maintain.
38. Insects, rodents, & animals not present
- comments: observed evidence of rodent activity on the premises. Observed approximately 15 mouse droppings on the floor of the utility room and rear dock where walk-in cooler and freezer are. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation #7-38-020(a). Citation issued.

- comments: observed high temperature dishmachine temperature gauge on dishmachine in room 136 not working properly. Instructed to repair and maintain with a wash temperature at least 160f and a sanitizing temperature of 180f. Priority foundation violation. #7-38-005. Citation issued.
