6. Proper eating, tasting, drinking, or tobacco use
- comments: observed personal drink item improperly stored inside refrigerator in the basement area. Instructed to designate a place for employee(s) personal food/drinks.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing sink in basement food prep area. Instructed to provide handwashing sink according to food code standards with hot and cold running water under city pressure along with providing handwashing signage, hand soap and paper towels. Priority foundation violation. 7-38-030(c). Citation issued.

36. Thermometers provided & accurate
- comments: observed no ambient air thermometers inside the reach in the rear cooler and inside the walk-in cooler and refrigerator in the basement area. Instructed to provide.

37. Food properly labeled; original container
- comments: observed food storage containers without labels containing the common name of foods. Instructed to provide.

39. Contamination prevented during food preparation, storage & display
- comments: observed food items stored in food containers in the rear prep area cooler and refrigerator in the basement area left uncovered. Instructed to provided food grade lids/covers to prevent contamination.

40. Personal cleanliness
- comments: observed food handler(s) prepping food without hair restraints. Instructed to provide/wear a hair restraint.

43. In-use utensils: properly stored
- comments: observed in-use utensils (tongs and knives) stored improperly on several prep surface areas. Instructed to store properly between usage.

55. Physical facilities installed, maintained & clean
- comments: observed exposed foam-like caulk on the floor at the opening door of the walk-in cooler in the basement. Instructed to correct.

56. Adequate ventilation & lighting; designated areas used
- comments: observed a missing light shield in the walk in freezer in the basement area. Instructed to provide.

58. Allergen training as required
- comments: observed certified food service sanitation manager with an expired allergen training certificate. Instructed the city of chicago food service sanitation manager must comply with completion of training and provide documentation.
