- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.
2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago food service manager certificate on site. While preparing and handling time and temperature control for safety(tcs) foods during inspection. Provided certificate is expired. Informed person in charge a valid city of chicago original food service certificate must remain posted on site and the certified holder shall be on site during tcs food prep, cooking, serving and operating hours. Priority foundation violation #7-38-012. Citation issued
38. Insects, rodents, & animals not present
- comments: noted evidence of pest activity on site. Noted approx. 15 small rodent droppings in storage closet located in entry way of kitchen. And 5 droppings on northeast wall under 2 compartment sink. Instructed to clean and sanitize affected areas. Priority foundation #7-38-020(a). Citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted all cutting boards in kitchen with deep cuts and stains. Instructed to restore to a smooth and easy to clean surface by sanding and sealing or replace and maintain.
51. Plumbing installed; proper backflow devices
- comments: observed no hot water at handwash sink in kitchen. Upon discovery, pic turned on hot water from shut off valve located between 2 and 3 compartment sinks in kitchen/dish area. Hot water at handsink and 3 compartment sink ran simultaneously. Instructed to repair. Hot water at each sink should work independently and on demand. Priority violation 7-38-030(c) citation issued.

- comments: noted encrusted debris throughout the floors in kitchen. Instructed to deep clean and sanitize.

- comments: noted clutter in northeast storage closet. Instructed to remove all unused unnecessary items for business. Kept items are to be stored on proper shelving to provide adequate cleaning of facility and to prevent pest harborage.

- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.
58. Allergen training as required
- comments: observed no food employee with proof of allergen training. Instructed all city of chicago certified food managers obtain this certificate.
