10. Adequate handwashing sinks properly supplied and accessible
- comments: noted hand wash sink by the dish washing machine inaccessible with cardboard box place ontop of sink as tray and food containers and a box of gloves. Instructed to have all hand washing sinks accessible at all times. Priority foundation violation #7-38-030(c). Citation issued.

- comments: noted no soap at the hand washing sink by the dish washing area. Instructed to have soap and paper towels at all hand wash sinks at all times. Priority foundation violation #7-38-030(c). Citation issued.

- comments: noted bulk food items stored inside containers at the rear food prep/dry storage not properly labeled. Instructed management that all bulk and working food ingredient containers must be labeled with contents name.

- comments: noted over 50 mice droppings along wallbase at the kitchen prep area, dish washing area, rear dry storage area, and on the second floor. Also noted 1 live roach on floor at the front bar. Instructed to detail clean and sanitize affected areas, eliminate pest activity. Instructed to consult with pest control company and have premises treated. Priority foundation violation #7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: noted individual to-go disposable containers and bowls with no handle used for scooping food at the service prep coolers. Instructed to provide tongs, serving spoons, or ladle for scooping food.

40. Personal cleanliness
- comments: noted employees at the prep area preparing food with no hair restraints or protection. Instructed to wear hair restraints or protection at all times

43. In-use utensils: properly stored
- comments: noted serving plates on shelves at the kitchen prep area and front dining area and dining table not properly stored. Instructed to invert all serving plates to prevent dust accumulation.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted assorted food items inside coolers and refrigeration units food stored inside grocery bags and tin cans. Instructed to properly store all food items inside approved food grade containers.

49. Non-food/food contact surfaces clean
- comments: instructed to detail clean the service prep cooler with spills on shelves, counter tops of the hot holding containers, all cooking equipment and around the service coolers with dirt, food particle, grease. Must also detail clean and maintain the inside of the electrical unit of the deep fryers.

- comments: noted leaking pipe at the bottom of the 3 compartment sink. Instructed to repair and maintain.

- comments: instructed to detail clean, sanitize and maintain floors throughout establishment especially at the areas where mice dropping and roaches were found.

- comments: noted certified food managers on duty with no allergen training certification. Instructed management that all food service managers are required to have the allergen training certification.

- comments: noted the following previous core violation #39,#56,#57 from inspection report #2620917 dated 7/18/2025 not corrected. ---39)3-307. 11: observed large pots and pans stored on the floor in the prep area underneath prep sink and handwashing sink. Instructed to elevate cooking equipment to protect from contamination. ---56)6-501. 14 observed heavy grease accumulation in ventilation hood above cooking equipment in the frying area. Instructed to detail clean and maintain. ---57)2-102. 13: observed expired food handler certificates for food employees on site. Instructed to recertify/renew food handler training and keep certificates on site. Instructed management that all food service managers are required to have the allergen training certification. Priority foundation violation #7-42-090. Citation issued
