- comments: observed no handwashing signs at handwashing sinks in staff washrooms. Instructed manager to provide handwashing signs at all handwashing sinks used by employees.

- comments: observed numerous packages of meat spoiled, discolored (black, brown and green patches) and displayed for sale in meat display cooler. Instructed manager to discard. Food displayed for sale shall be safe, unadulterated and in good condition. Priority 7-38-005 citation issued

- comments: observed fly strips hanging in basement produce prep area. Instructed manager to remove all fly strips from prep area.

- comments: observed meat trays stored on floor inside meat walk-in cooler. Instructed manager to elevate all trays six inches off floor.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed water leaking and ice buildup on compressor unit inside meat prep area. Instructed manager to repair and maintain unit.
51. Plumbing installed; proper backflow devices
- comments: observed hose connected to sink in meat prep area with no backflow device. Instructed manager to remove and provide backflow device.
55. Physical facilities installed, maintained & clean
- comments: observed missing ceiling tiles in meat prep area. Instructed manager to replace ceiling tiles and maintain.
