10. Adequate handwashing sinks properly supplied and accessible
- comments: observed improper maximum water temperature of 158. 9f at handwashing sink in women's washroom. Instructed a handwashing sink shall be equipped to provide water at a maximum temperature of 120f through a mixing valve or combination faucet. Priority foundation violation 7-38-030(c) citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed pink and black moldlike substance on upper interior compenents of ice machine in basement. Instructed to destroy ice, clean and sanitize interior of ice machine to prevent contamination of ice in ice machine and maintain.

- comments: observed no metal stem thermometer on premises. Corrected on site metal stem thermometer utilized to ensure attainment and maintenance of food temperatures delivered to facility before conclusion of inspection. Priority foundation violation 7-38-005 citation issued

- comments: observed opening at base of outer front entrance door. Instructed to seal all outer openings to prevent pest entrance.

- comments: observed spoons, knives and forks stored laying down in uncovered container on shelving underneath pos system. Instructed to store utensils inverted or covered during storage to prevent contamination.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed containers of food stored on floor in both interior and exterior walk in coolers. Instructed to provide adequate shelving units to elevate all food and nonfood items at least six inches off of floor for ease cleanability. Observed bag of spices stored on floor and in basement dry storage area. Instructed to provide adequate shelving units to elevate all food and nonfood items at least six inches off of floor for ease cleanability. Observed pallets used to elevate cases of drinks in basement dry storage area. Instructed to eliminate usage of pallets for elevation, instructed to provide adequate shelving units to elevate all food and nonfood items at least six inches off of floor for ease cleanability. Observed plastic film on equipment throughout facility. Observed plastic film on hot holding unit and cold holding units throughout first floor in prep areas. Instructed to remove plastic film from all equipment throughout facility. Nonfood contact surfaces shall be constructed of a corrosion resistant, nonabsorbent and smooth material.

- comments: observed no quaternary ammonium test kit on premises. Corrected on site test kit that accurately measures the concentration in mg/l of quaternary ammonium sanitizing solution delivered to facility before conclusion of inspection. Priority foundation violation 7-38-005 citation issued

- comments: at time of inspection facility provided expired or no allergen awareness training certificates for employees with city of chicago food service sanitation certificates. Instructed to provide valid proof of allergen training for all employees with city of chicago food service sanitation certificates.
