- comments: observed 150 mouse droppings scattered throughout the facility in the following areas; behind tables, buckets, in doorway leading to exterior apartment, and throughout basement area behind tables. Instructed person in charge to remove all mouse droppings, sanitize areas and inform pest control of need for additional service. Priority foundation #7-38-020(a). Citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed clutter behind tables in prep area, and in basement area on floor. Instructed person in charge to organize and maintain all items at least six inches from floor at all times and maintain.
57. All food employees have food handler training
- comments: observed no food handler training certificates for food handler employees on site. Instructed person in charge to provide food handler certificates.
