- comments: 2-102. 14(o) observed no employee health policy on site. Instructed to provide at all times. Priority foundation. Citation issued 7-38-010.

6. Proper eating, tasting, drinking, or tobacco use
- comments: 2-401. 11 observed the certified food manager smoking a cigarette in the food prep area. Instructed to only smoke outside of the facility and never smoke inside of the facility.

10. Adequate handwashing sinks properly supplied and accessible
- comments: 6-301. 12 observed no handdrying provision at the handwashing sink that is located next to the 3 compartment sink. Observed no handdrying provision provided at the cooking line handwashing sink. Instructed to provide a handdrying provision, such as paper towels at all handwashing sinks at all times. Employee provided papertowels during the inspection. Priority foundation. Citation issued 7-38-030(c).

- comments: 6-301. 11 observed no handwashing cleanser located at the handwashing sink at the cooking line. Instructed to provide handwashing cleanser at all times. Employee provided handwashing cleanser during the inspection. Priority foundation. Citation issued under violation above. 7-38-030(c).

- comments: 7-102. 11 observed containers of chemicals in containers without the common name of the chemical on the container. Instructed to label all chemical containers that are not in the original packaging. Priority foundation. Citation issued 7-38-015.

- comments: 3-302. 12 observed dry storage containers without labels containing the product name. Instructed to label all dry storage containers.

- comments: 2-402. 11 observed the male food handler without a hair restraint. Instructed to provide at all times.

- comments: 3-304. 14 observed soiled wiping cloths on the attached cutting board and not in sanitizing solution. Instructed to always store wiping cloths in a sanitizing solution at all times.

- comments: 4-502. 13 observed cardboard being reused as a floor liner in the toilet room. Instructed to remove and do not reuse cardboard.

- comments: 4-601. 11(c) observed the exterior and interior of all cooking and refrigeration equipment with accumulated food and grease debris. Instructed to clean all.

- comments: 6-501. 12 observed accumulated food grease and dirt debris on the floors under the cooking equipment. Observed excessive accumulated grease debris under the flattop grill. Observed accumulated dust and grease debris on the vents above the cooking equipment. Instructed to clean all.

- comments: 8-304. 11(a) observed the most previous inspection report summary not visible for the public's view. Instructed to always make the most recent inspection report summary visiable to the public at all times. Priority foundation. Citation issued 7-42-010(b).

62. Compliance with clean indoor air ordinance
- comments: 7-32-015 observed the food sanitation manager smoking a cigarette in the food prep area at the beginning of the inspection. Manager quickly hid the cigarette upon inspector's observation. Observed cigarette ashes on the floor where the manager was smoking the cigarette and cigarette ashes in the toilet room. Instructed to never smoke inside of public places. Smoking inside of public places is prohibited in the city of chicago at all times. Citation issued 7-32-015. Second offense.
