1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed person in charge without city of chicago food service sanitation certificate.

2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food service manager onsite while tcs foods are being prepared and served such as chicken, rice, beens etc. Instructed above to operate with a city of chicago certified food service manager at all times food is being prepared and served. Priority foundation violation 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed ho hot water available at the handwashing sink in the unisex employee washroom. Water temperature 65. 9f at this time of inspection. Instructed to repair and maintain hot water at the minium temperature of 100. 0f for proper hand washing. Facility has another handwashing sink with close proximity for employes to wash their hands. Person in charge turn on water tank pilot. Hot water has been repaired by the end of inspection. Priority violation 7-38-030(c). Citation issued.

- comments: observed no paper towels provided at the exposed handwashing sink located next to prep sink in the kitchen food preparation area. Person in charge provided paper towels during inspection. Priority foundation violation 7-38-030(c). No citation issued. Consolidated with above violation.

- comments: observed no handwashing signs or posters near exposed handwashing sinks in front bar area and in rar kitchen food preparation area. Must provide hand washing signs or posters.

- comments: observed food items stored on floor in rear kitchen food preparation area. Must elevate items 6 inches of the floor to prevent contamination.

41. Wiping cloths: properly used & stored
- comments: observed wiping cloths stored on top of prep tables in rear kitchen food preparation area. Must properly stored wiping cloths in a sanitazing solution to prevent contamination.

43. In-use utensils: properly stored
- comments: observed ice scoops stored inside ice bins in the front bar area. Must properly store ice scoops in a clen container to prevent contamination.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed raw wood wall underneath exposed handwashing sink in rear dishwashing area. Must be sealed covered to be smooth and cleanable.

49. Non-food/food contact surfaces clean
- comments: observed excessive amount of ice build up inside uprigh tall freezer in rear food storage area. Must remove ice and clean interior of freezer.

51. Plumbing installed; proper backflow devices
- comments: observed slow leaking pipe from underneath exposed handwashing sink in rear kitchen food preparation area. Must repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed the employee bathroom door at the rear prep/food storage area not self closing. Instructed to provide a self door closing device and maintain at all times

55. Physical facilities installed, maintained & clean
- comments: observed broken and damaged floor tiles in both washrooms. Must repair or replace. Observed dirt and grease build up on floor under, around and behind all cooking equipment and refrigeration units. Must clean and maintian.

- comments: observed clutter and unnecessary items inside the employee washroom. Must remove clutter and unnecessary items to prevent rodent harborage. Observed unused 2-door cooler in main kitchen food prep area. Must repair or remove from premises.

- comments: must provide food handler training for all the require employees and maintain certificates on site.

- comments: observed no proof of allergen training for city of chicago certified food service managers. Instructed person in charge any person with the city of chicago food service sanitation certificate must obtain allergen training certificates.

60. Previous core violation corrected
- comments: observed previous core violations not corrected from inspection report#2627005 on 11/13/2025 #37. Observed bulk dry food storage containers in the rear dry storage area not labeled with the name of the food stored. Instructed to label all bulk dry food containers with the common name of the food stored and maintain at all times. #49. Observed food- and nonfood-contact surfaces of large oven in the main kitchen area to be heavily soiled with encrusted grease deposits. Instructed to clean food- and nonfood-contact surfaces of cooking equipment with adequate cleaning materials and at adequate frequency to prevent build up of encrusted grease deposits and maintain. Priority foundation violation 7-42-090. Citation issued.
