16. Food-contact surfaces: cleaned & sanitized
- comments: observed a black slimy substance build up on the interior panel of the inside of the ice machine in the basement food prep area and made contact with the ice inside the unit, and the ice is used for human consumption. Must remove build-up, clean, and sanitize. Advised not to use the ice in the ice machine unit. Priority foundation 7-38-005, citation issued.

38. Insects, rodents, & animals not present
- comments: observed approximately 15 rats dropping on the floors at the basement dry storage area under the shelving unit where the aluminum foil-to-go containers are stored (10) and in the boiler storage area also located in the basement (5). Instructed to clean and sanitize affected areas. Recommended to consult with pest control company. Priority foundation #7-38-020(a), citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed two loaves of bread stored behind the faucet head at the one compartment sink in the main food prep area, near the deep-frying units and handwashing sink. Instructed to properly store food items and maintain.

49. Non-food/food contact surfaces clean
- comments: observed the shelving units in the walk-in cooler in the basement with slight rust and residue build up. Instructed to remove rust and soiled residue and clean and sanitize said shelving units and maintain, so that the shelving is smooth and easily cleanable.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed the outdoor grease receptacle with dried up grease droplets and not maintaied. Instructed to clean and maintain said outdoor receptacle and maintain.

58. Allergen training as required
- comments: observed no allergen training certificate for the manager on duty. Instructed that only certified food managers must take the allergen training course and maintain records on site at all times.

60. Previous core violation corrected
- comments: previously core violation was not corrected from 09/10/2021 report #2531249. #51 noted leakage at the bottom pipe of the hand wash sink in the basement food prep area. Instructed to repair and maintain. #55 instructed to clean floors along the wall base and underneath equipment, especially at the basement food prep/dry storage area. Priority foundation:7-42-090, citation issued

not eat breakfast or anything else prior to the whopper. I drank bottled water at the location, drank the same filtered water as my fiancé at home this morning, and only ate a few fries (which my fiancé also ate, and he’s currently ok). No food since. I’m a molecular biologist with a focus on infectious diseases, and I know what to look for regarding food poisoning cases.

Update 30/03: I actually suspect it's a toxin-producing bacterium, such as *S. aureus*, especially considering the rapid onset of symptoms. We also don't have a high number of norovirus cases in our area, so it's not something that I would consider to be suspicious when weighing other factors like norovirus' incubation period, symptoms like fever and myalgia, and rapid spread. I'm also not in close contact with many other people or shared surfaces, further reducing the index of suspicion. On the other hand, *Staph aureus* is ubiquitously found and the enterotoxins produced are highly heat stable, making it more likely to be the causative agent if the meat was older, improperly handled (especially regarding hygiene practices) or stored, or not cooked to temperature. The more aggressive symptoms are beginning to subside, though I'm definitely not the happiest-of-campers at this time.  | Symptoms: Nausea, Diarrhea, Vomiting, Stomach Pain, Lightheaded
