degrees and buckets of cut lettuce were measured 46 degrees – both of which were above the 41-degree maximum.
- The ice machine was adulterated by a “heavy buildup” of brown slime on the interior edges, and the food dicer was crusted with dried food and debris.
- Not all of the workers designated as “in charge” were certified food protection managers, and that the handwashing sink in the kitchen was out of service with no hot- or cold-water supply.
- Flying insects are noted throughout the establishment, food storage and equipment-storage areas. Rodent droppings are observed on shelving in the food-storage areas. Insects were observed in the spice-salt mix used for mixed drinks at the bar.
- Rolling cart, with open containers of raw chicken on it, sitting in “water pooled on the floor from backed-up floor drains.”
- Clean equipment and utensils were being stored in an area where they were exposed to splashing from the standing water that had pooled on the floor.
- Some of the kitchen walls were in poor repair and ceiling tiles were missing. Some of the holes in the wall had been repaired with tape that was peeling, and the floors, equipment and shelving each had a “large amount of spilled food, debris, and buildup” on them.
- Light bulbs in the ventilation hood systems are not shielded and have a large amount of dripping grease and residue.
The inspector returned on Nov. 17 and reported finding live and dead insects inside containers of dried spices, bouillon powder and sugar, and made note of the “heavy presence of live cockroaches” in the kitchen’s food-preparation area and storage areas.
Due to the heavy presence of cockroach-like insects in the kitchen area, the person in charge stated that the establishment would remain voluntarily closed until pest-control applications could be completed.

I ordered a Bacon King (#4) after work yesterday (4/16), got home and slept early, but woke up by 10pm with diarrhea and vomiting. This lasted for some 3 hours till I got to sleep again by 1am. I got up next at 4am experiencing the same
issues in the bathroom, feeling lightheadedness and chills. I was also sweating after vomitting. Had to call out from work. Not sure if it was the bacon or the beef, or both. After the initial vomit, most of the contents of the vomit was bile, and I concluded that I had food poisoning.

Went to chik fil a yesterday and got my daughter a chicken sandwich and fries. It was the only thing she ate all day except oatmeal for breakfast. Few hours later she asked me to feel her head said she felt warm then threw up shortly after.
Then ran to the toilet with diarrhea. Shes thrown up 4 times and diarrhea twice since yesterday, feels fine otherwise. Just cant keep any liquids down for long before she throws up again.

Additional information: I had been drinking Ryze Mushroom Matcha for about a month and a half before the onset of daily episodes of
chills, fever and tremors, starting sometime between 10:30a and 1:30p. At first I was not sure of the relationship and sought treatment from my PCP who could not find any cause after examining me and in his office. During my second episode he sent me to the ER because my fever would not respond to ibuprofen. A battery of tests were done (urinalysis, CT scans of head, pelvis, and abdomen, chest x-rays, EKG, full blood work analysis) and no apparent cause was found, so I was put on a broad spectrum antibiotic as a precaution. I voluntarily stopped drinking the Ryze Mushroom Matcha for a few days, although my PCP and the ER doctor did not think it could be the cause, and I did not have the fever and tremors on the days I did not drink it. Once I felt totally recovered and was done with the antibiotics, I drank my usual cup of Ryze Mushroom Matcha, but only about half the cup. Within 4 hours after drinking it, I had another episode of severe chills, tremors, and a fever which finally subsided about 4 hours after taking 800mg of Motrin. It's taken a few days for my body to recover, but I have stopped drinking the Ryze Mushroom Matcha and I have not had any recurring episodes. The closest drop-down was Rhyze Mushrooms, but that’s not correct. I ordered the Ryze Mushroom Matcha from ryzesuperfoods .com.

I ate at Thai Smile on a Saturday 9/10/2022. I started feeling sick Sunday night , Monday sluggish and tired, Tuesday 9/13 developed headache and later in day severe diarrhea. Home sick with fever, chills, no appetite, Wed and Thursday. Thursday night 10 pm presented to Er
with dehydration, fast, labored breathing. Low blood pressure. andmitted anlmist sent to ICU but blood pressure recovered after 5 bags IV fluids. Acute sepsis. Was hospitalized 11 days. Bill $275,000.00 thank God for insurance. Out of work 30 days.
