An inspector ordered that Samurai Japanese Steakhouse and Sushi Bar be temporarily shut down on Aug. 2 after live insects were found in the establishment.
An inspector observed nine live roaches.
Observed approximately 100 insect larvae under a dishwashing machine.
Stick... See Morey tape for catching flies was hanging over a food preparation area.
Raw salmon was stored over pasta in a walk-in freezer. The pasta was relocated.
Chemicals were stored with food on the cook line.
Medicine was stored with food on the cook line. An employee moved the medicine.
A bucket of dried shrimp was stored on the floor of a walk-in freezer. A manager shelved it.
A cutting board on the cook line had cut marks and was no longer cleanable.
There was grease accumulated under cooking equipment, on walls and on the cook line.
Raw shrimp was thawing at room temperature. The inspector educated a manager regarding proper thawing techniques, and the shrimp was moved to a reach-in cooler for thawing.
Cut vegetables were stored in reused boxes in a walk-in cooler. Corrective action was taken.
Surfaces in a walk-in cooler were soiled, according to an inspector.
There was an encrusted material on a can opener blade. An employee cleaned it.
The restaurant passed a follow-up inspection on Aug. 3 and was allowed to reopen.

I have a headache as well!! I called them the customer service person told me that they were having problems with their produce lately and the produce manager was aware of it that makes me very angry that he knows there's a problem he obviously knows about the orange problem in the diseased trees well I don't care what anyone says it made me sick I Read all about it so even after washing my hands thoroughly I walked into the other room and my mother said you smell like bitter and sour that's because it was still on my hands and the bag of oranges on the kitchen counter smell terrible and they have not been peeled at any rate I'm bringing or my step brother is bringing the oranges back the customer service person was wonderful and felt terrible about situation but the fact that the produce manager is aware of the issue and still putting out produce that's bad is upsetting to me I just can't even believe someone would do that my question is... Is they buying oranges from The underground orange market it sounds ridiculous but I'm sure there is one ... Sorry if I'm not working this the best way I just am so sick right now I feel like I'm going to throw up!! I'm legally blind and on disability I don't have money to be spending on rotten fruit that's going to make me sick or anything else for that matter who knows is there meat infected?? My stepfather got me a bag of oranges the small ones i8 almost half the orange after washing it I got sick about an hour after eating although I didn't feel too well it was bitter and sour almost pesticidas tasting... I feel very sick I threw up my head hurts I'm nauseous I have this horrible taste in my mouth that will not go away and diarrhea I just feel all over my body like I have the flu and I'm the only person that ate an orange thank God my mother didn't have one!!

FSIS advises consumers who have purchased these products not to consume them. These products should be thrown away or returned to the place of purchase.

The products subject to recall do not bear a Federal mark of inspection. These items were shipped to distributors, retail locations, and restaurants in Arizona, California, Hawaii, Nevada, New York, and Texas. The following heat-treated shelf-stable Hotpot Seasoning items containing beef tallow are subject to recall:

- 17.6-oz. vacuumed sealed packages of “Ming Yang Hotpot Seasoning (Slightly spicy).
- 17.6-oz. vacuumed sealed packages of “Ming Yang Hotpot Seasoning (Super spicy, Extremely).
- 12.07-oz. vacuumed sealed packages of “Ming Yang Hotpot Seasoning (Medium spicy, Mala).

See Morermesan-Crusted Wild Alaskan Salmon Fillets, net wt.12oz, frozen, UPC 41415 30245 7, because it may contain undeclared soy due to mis-packaging. People who have an allergy or severe sensitivity to soy run the risk of serious or life-threatening allergic reaction if they consume this product.

The affected Parmesan-Crusted Wild Alaskan Salmon Fillets retail box bears the code date “Best Before/Use By date 021122”, and the affected Teriyaki sauced product is observed through a clear inner vacuum package inside the retail box. Images of the affected and non-affected product are found below. The affected product was distributed directly to Publix Super Markets Inc distribution warehouses located in Florida and Georgia between 9/18/20 and 11/4/20. Publix further distributed product to their various retail stores in the southeast.

The problem was discovered when a consumer reported that the Teriyaki sauced salmon was found inside the retail box labeled as Parmesan-crusted salmon. Further investigation by the co-packer UniSea Cold Storage of Redmond, WA revealed that the that the loading box machine operator must have inadvertently grabbed the Parmesan-crusted salmon retail box instead of the correct boxes. We estimate only 300 units of 8450 Teriyaki sauced salmon packages were mis-packaged as Parmesan-Crusted Wild Alaskan Salmon Fillets.

There have been no adverse/allergic reactions reported to date. This recall is being made with the knowledge of, and in cooperation with, the U.S. Food and Drug Administration. Consumers that find any affected product as described in this recall notice are asked to either dispose of the product or return the product to the point of purchase for a refund. “I bought some sliced butterball turkey from their meat butcher on January 26 for 15$. On Feb 12 I ate 2 of these slices, thickness 1 1/2 in a sandwich (bread, pickle, tomato, and some lettuce along with some other food (egg & cheese again from there), after about 5 hours later I got sick.

I did... See Morenot count the number of times I pooped. I also did not sleep very well that night, and had to get up and pace around my apartment from the discomfort I felt. At about 3AM the discomfort reached its peak and very slowly subsided. At about 7AM I was feeling much better.

I could not bring myself to through out all the food (the turkey + the egg and cheese alone was over 35$) that I had eaten from.

After only having a liquid diet for 24 hours, On Feb 13, I again had the same turkey sandwich but without the tomato. This time I also had a brownie that was given to me by a neighbor. This time I got sick after only about 3 hours. Now I am sure it was the turkey. This time it was a little worse, and after much pooping, I called the health nurse at the at the VA about 1AM Feb 14. She advised me to drink plenty of liquids and wait until morning to see how I felt.

I again had a very agenizing night again pacing around my apartment because I could not bring myself to do anything else. At about 2AM I vomited 4 times in quick succession. I was extremely glad to get that crap out of my stomach but didn't feel all that better after doing so as I had expected it to.
The extreme discomfort again slowly went down and about 8 AM I was feeling ok.

On Feb 17 I went to the store to return the meat but forgot to bring it. I found out that they had been closed for 3 days because of having no power. I needed to buy some more food so went to the frozen food section.

I found most of the shelves very low on frozen food. I talked to one of the workers about that, that was restacking the frozen food, and she said since they had lost power for 3 days they had to throw out all the frozen and refrigerated food. I chatted with her and another customer for a little while. I again asked her if she was sure all the frozen and refrigerated food was thrown out and she said she was.

I bought 3 Michelina's Chicken Fried Rice dinners and 1 Marie Callender's Chicken Pot Pie. I went home and immediately ate 2 of the Fried Rice dinners, and again got sick within about 4 hours. Again I have a sleepless night with extreme discomfort, but not as bad as the second time I got food poisoning, and again in the morning I felt better.

It is my belief that they didn't throw out all the frozen food. Frozen food that they thought would make it through the 3 day outage such as rice would be ok.

On Feb 21 I took back the turkey there and got a refund. I did not take back the rice for 2 reasons. First they are only a 1$ apiece, and 2nd I didn't want it to look like I was scamming them.

Proving food poising looks to me like a uphill battle, unless there are others like me that have been poisoned. My 3rd case of food poisoning on the other hand may not be as hopeless as it seems. If they did not throw out all the frozen and refrigerated food as they said it is a different matter, after all their own surveillance cameras may show a different story! I am also sure that the people that insure they would like to know if all the food they claimed was thrown out was in fact done.

I still have yet to throw out 1 unopened Chicken pot pie, and 1 Chicken Fried Rice and 2 opened Chicken Fried Rice dinners. The codes on those dinners may show that they were thrown out with the insurance company. As for now, I don't think I will ever buy frozen or refrigerated food there again.
