- comments: observed homemade yogurt being made onsite without an approved variance/haacp plan. Found pot of yogurt stored in a walk in cooler during the inspection. Instructed that a variance/haacp plan must be first approved by cdph, before yogurt can be made onsite. Yogurt discarded. Priority foundation 7-38-005 citation issued.

- comments: found wiping cloths stored improperly in prep area. Must store wiping cloths in sanitizing solution between uses to prevent contamination. Instructed to maintain.

- comments: found previous priority foundation violation not corrected from inspection report dated 8/18/20. Violation includes: #38: observed approximately 30 live roaches on prep tables, walls, and floors, in multiple prep areas of facility. Instructed to remove roaches, clean and sanitize the affected areas, and contact pest control for service, and maintain. Priority foundation 7-38-020(a) citation issued. Observed 28 live roaches onsite during the re-inspection. Violation still exsist. Priority violation 7-42-090 citation issued.
