- comments: observed build up of grease, grime, food debris, and soiled cardboard in cabinet underneath soft drink machine. Also observed dirty toilet bowl in restroom. Instructed to remove cardboard, detail clean, and maintain in good condition and detail clean toilet bowl and maintain in good condition.

- comments: observed no allergen training certificates on site at time of inspection. Instructed all employees holding city of chicago foodservice manager certificate must complete allergen training and maintain records thereof on site at all times.
