Report date: Apr 8, 20265. Procedures for responding to vomiting and diarrheal events
- comments: observed all necessary items with written procedure not available on site when employees respond to vomitting or diarrheal events. Instructed to obtain and maintain on premises all necessary items and written procedure for employees to follow when responding to vomitting or diarrheal events. Priority foundation violation 7-38-005 citation issued
38. Insects, rodents, & animals not present
- comments: observed openings in middle of main entrance double glass doors, observed openings at lower right corner and side of rear exit door leading from prep area and observed opening at lower left corner and right side of rear exit hallway door. Instructed to seal all outer openings to prevent pest entrance.

- comments: observed approximately 120 mouse droppings in facility. Observed mouse droppings in prep/dishwashing area, dry storage areas and in bar. Instructed to eliminate rodent activity, remove mouse droppings and clean affected areas. Recommended licensed pest control company service facility on or after 4/8/2026. Priority foundation violation 7-38-020(a) citation issued
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no stoppers for three compartment sinks in facility. Instructed to provide appropriately sized stoppers for all three compartment sinks in facility.
55. Physical facilities installed, maintained & clean
- comments: observed mop stored in bucket. Instructed to install mop drying rack for proper drainage and drying of mops to prevent pest breeding.

- comments: observed openings in between lower walls and floor in rear hallway, storage room behind bar and storage room with table and chairs. Instructed to smoothly repair all openings in said area for ease of cleanability.
57. All food employees have food handler training
- comments: at time of inspection facility provided expired food handler certificate for food handler. Instructed to provide valid food handler certificates for all food handlers.
