Report date: May 8, 20265. Procedures for responding to vomiting and diarrheal events
- comments: observed all necessary items when following written procedure not available on site when employees respond to vomitting or diarrheal events. Instructed to obtain all necessary items for following written procedure for employees to follow when responding to vomitting or diarrheal events. Priority foundation violation 7-38-005 no citation issued
38. Insects, rodents, & animals not present
- comments: observed approximately 70 mouse droppings and 15 live ants in facility. Observed mouse droppings in small and large dry storage areas of facility. Instructed to eliminate rodent activity, remove mouse droppings and clean affected areas. Observed 15 ants in window in room 128. Instructed to eliminate ant activity and clean affected areas. Recommended licensed pest control company service facility on or after 5/8/2026. Priority foundation violation 7-38-020(a) no citation issued
51. Plumbing installed; proper backflow devices
- comments: unable to locate backflow prevention device on ice machine. Instructed to install backflow prevention device on ice machine so that it shall be located so that it may be serviced and maintained.

- comments: observed leaking faucets at handwashing sinks in washrooms in prep area. Observed leaking hot water faucet in ladies washroom and observed leaking hot and cold water faucets in men's washroom in prep area. Instructed to repair said faucets in said washrooms and maintain. Observed no hot or cold water pressure at left handwashing sink in girl's south second floor washroom. Observed low water pressure at left handwashing sink in second floor boy's south washroom. Observed no cold water pressure at handwashing sink in third floor all gender staff washroom. Instructed to restore water pressure to said handwashing sinks in said areas and maintain.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed damaged lids on outside trash and recycling dumpsters. Observed damaged lids on two trash dumpsters and one damaged lid on one recycling dumpster, damaged lids create openings on said dumpsters. Instructed to replace or damaged dumpster lids and maintain tight fitting lids on dumpsters. Priority foundation violation 7-38-020(b) no citation issued
