- comments: observed all necessary items when following written procedure not available on site when employees respond to vomitting or diarrheal events. Instructed to obtain all necessary items for following written procedure for employees to follow when responding to vomitting or diarrheal events. Priority foundation violation 7-38-005 no citation issued

22. Proper cold holding temperatures
- comments: observed cold tcs foods, held at improper temperatures ranging between 46. 6-48. 7f. Observed cold tcs foods, salad with chopped tomatoes(46. 6-48. 7f) and chopped tomatoes(47. 5-48. 0f) held at improper temperature. Instructed all cold tcs food must be held at 41f or below. Corrected on site manager voluntarily discarded and denatured food valued at $5 weighing 2lbs priority violation 7-38-005 no citation issued

38. Insects, rodents, & animals not present
- comments: observed pest activity in facility. Observed approximately 5 mouse droppings in facility. Observed mouse droppings in walk in dry storage. Instructed to eliminate rodent activity, remove mouse droppings and clean affected areas. Observed 5 ants on floor in between wall and shelving unit outside of walk in cooler. Instructed to eliminate ant activity and clean affected areas. Recommended licensed pest control company service facility on or after 4/15/2026. Priority foundation violation 7-38-020(a) no citation issued

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed crates used to elevate boxes in the walk in freezer and walk in cooler. Instructed to provided adequate shelving units to elevate all food and nonfood items at least six inches off of floor for ease of cleanability.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in all gender employee washroom in rear of prep/dishwashing area. Instructed to provide covered receptacle in all gender employee washroom in rear of prep/dishwashing area for sanitary napkins.
