3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: unable to produce the employee health policies. Instructed to provide employee health policies for all employees verifying acknowledgement of the policy. Priority foundation violation 7-38-010 issued for food handler requirements.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the handsink floor drain draining slow; instructed to correct so that sink drains properly. Priority foundation violation 7-38-030(c) citation issued for compliance with city rules.

16. Food-contact surfaces: cleaned & sanitized
- comments: found no chemical sanitizer at low temp dishmachine. Chemical test strip reading 0 ppm at low temp dishmachine after a full cycle. Instructed to correct to read at 100 ppm. Priority violation 7-38-025 citation issued for equipment standards.

- comments: observed the interior of the ice machine in the rear of the kitchen with a slimy substance inside. Instructed to clean and maintain. Priority foundation violation 7-38-005 issued for food requirements.

- comments: must label all bulk storage containers with name of product inside of container.

- comments: rodent infestation observed on premises. Found 400 mouse droppings on premises in the kitchen on the floor around under and behind various equipment, on top of dishmachine, on top of prep table, inside the bottom area of the fryer, under storage shelves in rear area by walk-in units, on the floor in the storage area inside of kitchen, on the floor in the storage area and furnace area outside of kitchen, on shelves behind the beverage counter and in the closet by the beverage cooler. Instructed to clean all areas and have pest control company service establishment. Priority violation 7-38-020(a) citation issued for control of vermin and insects. Observed 4 live roaches on premises and a dead mouse.

- comments: observed wiping cloths at prep table not stored inside of a sanitizing solution while not in use; instructed to correct.

- comments: instructed to clean and maintain the following: shelves inside of the walk-in cooler, reach in coolers, exterior surfaces of fryer to remove grease, wok station area, prep tables and all other kitchen equipment.

- comments: observed floors, walls, ceiling tiles and vents in need of detail cleaning in kitchen, dining area and storage areas; clean and maintain.
