- comments: 3-302. 12 observed some dry storage containers without the common name of the food on the container and spices out of the original containers without the name of the spice on the storage container located by the wok station. Instructed to provide proper labels for all foods out of the original packaging.
38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 30 rodent droppings in the basin of the utility sink that is located in the rear, about 30 rodent droppings in the far right corner under the 3 compartment sink, about 30 rodent droppings on the dry storage shelves on top of the containers stored on the shelves, about 20 rodent droppings under the customer service counter on the lower shelves and about 20 rodent droppings on the lower shelf of the prep table that is located by the smoothie prep cooler. Instructed to eliminate pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 3-304. 12 observed bowls used to scoop food from the container being stored directly into the container. Instructed to not store any utensil to scoop foods inside of the food containers.

- comments: 6-201. 13 observed wall coving missing in various areas under the 3 compartment sink. Instructed to replace coving and make said areas smooth and easily cleanable.
57. All food employees have food handler training
- comments: 2-102. 13 observed 3 out of 6 food handlers without proof of food handler training. Instructed to meet requirement for all food handlers.
58. Allergen training as required
- comments: 2-102. 13 observed the food service sanitation managers on site without the allergen training certificate. Instructed to meet requirement for all food service sanitation managers.
