- comments: no thermometer in the prep cooler. Instructed to obtain and maintain a working thermometer in all coolers.

- comments: rodent activity found on the premises. 40 in the front prep area, 80 under the pop crates and storage racks in the front sales area and 60 in the back by the hot water tank. Instructed to remove all droppings, clean and sanitize all affected areas. Priority foundation 7-38-020(a) citation issued
51. Plumbing installed; proper backflow devices
- comments: front 2-compartment sink (one compartment is used as a hand sink) slowly drains. Instructed to fix and maintain good plumbing.

- comments: water-stained ceiling tiles in the front sales area. Instructed to replace ceiling tiles and maintain. No baseboards in the front prep area and the rear dish washing area. Instructed to provide baseboards throughout as needed. Maintain same.

- comments: found the previous core violations from report 2613783 dated 03/14/2025 not corrected and/or maintained. Instructed to correct and maintain the following: -10. Observed no hand washing sign at hand washing sink in dish washing area. Instructed manager to provide hand washing signs at all hand washing sinks used by employees. -47. Observed raw wood surfaces behind utility sink. Instructed manager to seal all raw wood surfaces and maintain. -47. Observed crates used as shelving in prep area. Instructed manager to remove all crates and provide adequate shelving for storage. -49. Observed food debris, spills and grease buildup on interior and exterior surfaces of fryers, slicers, prep tables, holding units, coolers, shelving units, beverage machines. Instructed manager to clean and maintain. -51. Observed no backflow device on utility sink. Instructed manager to provide and maintain. -55. Observed food spills and debris on walls in prep area, on floor along walls and corners in prep and storage areas. Observed dust buildup on ventilation fan in washroom. Instructed manager to clean and maintain. -55. Observed unused articles and equipment in rear storage and basement areas. Instructed manager to remove all unnecessary items to prevent pest harborage. Priority foundation 7-42-090 citation issued
