- comments: 2-102. 14(o) observed no employee health policy on site. Instructed to provide at all times. Priority foundation. Citation issued 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: 2-501. 11 observed no adequate supplies on site to properly respond to vomit and diarrhea events that can potentially occur inside of the facility. Instructed to provide at all times. Priority foundation. Citation issued 7-38-005.

23. Proper date marking and disposition
- comments: 3-501. 17 observed tcs foods (cooked chicken, cooked pork, cooked noodles) in refrigeration units, held for over 24 hours and not properly labeled with the discard dates. Instructed to provide labels with discard dates for all ready to eat tcs foods. Priority foundation. Citation issued 7-38-005.

- comments: 6-501. 111 observed evidence of pest activity on site; observed one live rodent running behind the refrigeration units in the food prep area. Observed over 100 rodent droppings scattered in various areas of the floor in the food prep area. Instructed to eliminate live rodent activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 3-304. 12 observed utensils being inadequately stored in a container of food. Instructed to always store all utensils properly at all times.

- comments: 4-601. 11(c) observed door handles on the refrigerator units with an accumulated debris. Observed stained cutting boards. Instructed to clean all refrigeration door handles and/or replace or clean and sanitize cutting boards.

- comments: 6-201. 11 observed excessive dirt debris on the floors throughout the food prep area and in the toilet room. Instructed to clean.
