- comments: observed wiping cloths stored on top of prep tables and 3-compartment sink. Must properly store wiping cloths in a sanitizing solution when not use.

- comments: observed ice scoops stored inside of ice machine. Must properly store ice scoops in a clean container to prevent contamination.

- comments: observed leaking faucet base at the 3-compartment sink in rear prep area. Must repair and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed dirt and debris on walls inside rear utility room. Must clean and maintain.

- comments: observed dust build up on ceiling ventilation vents in customers washrooms. Must clean and maintain.

- comments: previous core violations not corrected from 01/13/2021 inspection#2356553 #10 found no hand washing signage for hand sinks in food prep areas. Instructed to provide. #38 found front entrance doors not pest proofed approx:1/4' inch mode of entry at bottom of door. Instructed to make doors tight fitting all across the bottoms to prevent pest entry. #56 instructed to detail clean kitchen exhaust hood/filters to remove accumulation of grease build-up and to maintain priority foundation violation 7-42-090.
