- comments: observed rear exit door with small gap between door and floor. Instructed to provide a rodent proof door to prevent pest entry and maintain.
38. Insects, rodents, & animals not present
- comments: observed approximately 70 small rodent droppings on premise is the following areas. . In employee /storage room in corner behind door, under wooden scattered on floor, behind 2 door coolers/ freezers in dry storage area, surrounding ventillation/heating system and storage racks containing loaves of bread, and in office surrounding desk and camera system. Instructed to contact pest control for additional service, remove droppings, clean and sanitize said areas. Must eliminate rodent activity on site. Priority foundation violation. #7-38-020(a). Citation issued.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed receptacle inside unisex toiletroom without covering, instructed to provide and maintain.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed receptacle area outside with buckets and miscellaneus items stored behind grease receptacle. Instructed to remove and maintain refuse area in order to prevent pest harborage
55. Physical facilities installed, maintained & clean
- comments: observed ceiling missing with slight leak in office space as well as water stained /damaged ceiling tiles above cooling units/charcoal is stored. Instructed to repair and provide smooth and easily cleanable
58. Allergen training as required
- comments: observed 1 of 3 certified food manager certificate holders without allergen training. Instructed to provide and maintain on site for review.
