- comments: observed no employee health policy on site. Must provide written employee health policy for all employees. Priority foundation violation. 7-38-010.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed 3 high temperature dish machines not operational in main kitchen and on 2nd floor. All machines tagged and held for inspection.
49. Non-food/food contact surfaces clean
- comments: observed ice/water accumulation inside 2 door freezer in kitchen. Must repair/clean and sanitize. Must maintain same. Observed build up on bottom of hot holding unit (on floor) of kitchen. Must detail clean and sanitize. Must maintain same.
51. Plumbing installed; proper backflow devices
- comments: observed slow draining 3 compartment sink in bar area. Must repair and maintain same. Observed excessive build up on floor drain in kitchen under 2 compartment sink. Must detail clean and maintain same.

- comments: observed heavily soiled floors in dumpster/loading dock area. Must detail clean and maintain area.

- comments: observed previous core violations not corrected from report #2320579, dated 10/28/19. Must correct the following previously issued core violations: #47 observed a cutting board with deep grooves and stains in rear kitchen prep area. Instructed to repair or replace and maintain a smooth, cleanable surface. Observed a can opener in rear kitchen prep area covered with dirt and grime. Instructed to clean, sanitize, and maintain. #55 observed a build up of dust on ceiling panels in rear kitchen prep area. Instructed to clean, sanitize, and maintain. Observed dark brown stains in 2 compartment sink located in rear kitchen prep area. Instructed to clean and maintain. Priority foundation violation 7-42-090.
