- comments: observed employees eating in prep area. Instructed manager to provide designated area outside of prep area for employee meals.

- comments: observed front handwashing sink filled with multi-use containers and not accessible for handwashing. Instructed manager to remove all items from front handwashing sink. Handwashing sink shall be accessible at all times for proper handwashing. Priority foundation 7-38-030(c) citation issued

- comments: observed bulk containers not labeled in rear storage area. Instructed manager to label all bulk containers with common name of food.
38. Insects, rodents, & animals not present
- comments: observed over 30 rat droppings scattered on floor under syrup shelving unit and on floor and pallet near single service articles in basement storage area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed food items stored on floor in prep and storage areas. Instructed manager to elevate all food items six inches off floor to protect from contamination.

- comments: observed single service articles stored on floor in basement. Instructed manager to elevate all single service artilces six inches off floor.

- comments: observed no stoppers for 3-compartment sink. Instructed manager to provide adequate stoppers and maintain.

- comments: observed food debris and spills on intterior and exterior surfaces of handwashing sink, 3-compartment sink, prep sink, prep tables, shelving units, cooking equipment. Instructed manager to clean and maintain all equipment.

- comments: observed debris on floors under all equipment, along walls and in all corners in prep and storage areas. Instructed manager to clean and maintain.
