- comments: the employee on duty has no city of chicago food service managers certificate as required; must provide. Instructed that a city of chicago food service manager must be on duty at all times tcs foods are prepared and served. Priority foundation citation 7-38-012 issued.

- comments: observed no employee health policy on site. Instructed to provide. Priority foundation 7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed no written procedure nor adequate supplies on site to properly respond to vomit and diarrhea events that can potentially occur inside of the facility. Instructed to provide. Priority foundation 7-38-005.

- comments: observed a hand washing sign is needed at the food prep area hand washing sink. Instructed to provide.

- comments: observed no metal stem thermometer to properly take temperatures of cold and hot foods. Instructed to provide. Priority foundation 7-38-005.

- comments: observed rust on the shelves in the walk in cooler. Instructed to remove rust. Observed cardboard being re-used as a shelf liner. Instructed to not re-use cardboard and make all surfaces smooth and easily cleanable.

- comments: observed floors through out with accumulated dirt debris. Instructed to clean under all cold holding units, under dry storage shelves and under the 3 compartment sink.

- comments: previous priority foundation violation not corrected as required from march 26, 2021. #38 observed pest control log book and service report not available for inspector's review. Instructed to always have an up to date pest control log book with service report for the facility on site at all times. Priority violation citation 7-42-090 issued.
