- comments: observed no valid city of chicago foodservice sanitation certificate on site. Instructed person in charge that valid original city of chicago sanitation certificate must be on site at all times (photocopies and pictures not allowed). Priority foundation #7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hot water above 120f at the exposed handwashing sink next to the dishwashing sink in room #173(bento prep room). No cold water was provided to the exposed handwashing sink and hot water is observed at 140. 8f. Must provide hot water between 100-120f at the handwashing sinks. Priority foundation #7-38-030(c). Citation issued. (alternative handwashing sink available in room).

- comments: observed build up on interior of ice macine (not in use). Instructed person in charge to clean and maintain.

- comments: observed excessive leak at three compartment sink in room #174. Instructed person in charge to repair and maintain.

- comments: observed large holes in walls near rear dock area. Instructed person in charge to repair all damaged walls smooth and easily cleanable and maintain.
