- comments: observed no verifiable employee health and safety policy for all employees at time of inspection. Instructed manager to maintain a completed and signed employee health and safety policy for all employees on site. Priority foundation violation, 7-38-010. Citation issued.

- comments: observed large dry storage containers in preparation area to not be labeled with the common name of contents of container. Instructed manager to label all dry storage bins with common name of contents.

- comments: observed grease build up on drainage basin from dish machine. Instructed manager to clean and maintain area free of grease build up.

- comments: observed debris accumulation on the front bar drink nozzle holder. Instructed manager to clean, sanitize, and maintain the drink nozzle holder free of debris accumulation.

- comments: observed leak from 3 compartment sink drain when water drains in front bar area drain. Instructed manager to repair and maintain drain so as not to leak water onto floor.

- comments: observed food debris, dust build up, and trash along the walls in the basement and under the shelves next to the stairs leading to basement. Instructed manager to clean and maintain areas free of debris and dust build up.
57. All food employees have food handler training
- comments: observed no food handler certificates for all employees at time of inspection. Instructed manager to maintain records of food handler certificates for all new employees within 30 days of hire.
58. Allergen training as required
- comments: observed no up to date allergen training certificate at time of inspection for every certified food protection manager. Instructed manager to provide allergen awareness training for every certified food manager.
