- comments: observed food stored in containers in all the coolers, walk in cooler and dry storage without proper labeling of name. Instructed to person in charge to provide a label reflecting the food item's name when food has been remove from its original packaging.
39. Contamination prevented during food preparation, storage & display
- comments: observed food containers stored on the floor throughout premises in the walk-in cooler, prep and storage areas. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: observed food items stored throughout premises in main prep area, walk in coolers, cold-hold units, etc uncovered. Instructed to provided food grade lids and/or coverings when storing all food items to prevent contamination.

- comments: observed milk crates being used as shelving throughout the prep and storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed food splatter, buildup, and mold-like substance build-up along wall caulking throughout rear warewashing. Instructed to detail clean, sanitize, and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed clutter throughout premises especially in basement. Instructed manager to remove all unused articles and equipment not necessary for daily operation to prevent pest harborage.
60. Previous core violation corrected
- comments: noted previous core violations not corrected from report #2612449 conducted on 02/20/2025. They are as follows: #49. The following not clean- the interior of all prep coolers and freezers, floor mixer, gas lines behind the stoves, stoves and flat top grill, storage shelves, prep tables, can opener, exposed hand washing sinks throughout, food storage containers on the shelves of the kitchen, and interior and exterior of the microwaves, clean or replace the soiled bus trays used inside of the dish washing machine. Instructed manager must clean and maintain equipment. 55. Observed heavy debris on floors at rear stairwell and behind walk in freezer in basement and also near water heaters in basement supplies (to-go containers) storage area also observed food, spills and excess debris and build up on walls, floors, and splatter on ceilings at rear prep area and basement. Must clean and maintain floors and walls. Priority foundation violation #7-42-090. Citation issued.
