- comments: observed no valid city of chicago foodservice sanitation certificate on site during the preparation and service of tcs foods(milk, ground beef). Instructed person in charge that valid city of chicago sanitation certificate, as well as certified person, is required on site at all times tcs foods are prepared and served. Priority foundation #7-38-012. Citation issued.

- comments: observed hands removed from kitchen prep area. Instructed the person in charge that the handwashing sink must be re-installed with hot and cold running water under city pressure. Priority foundation #7-38-030(c). Citation issued.

- comments: observed excessive build-up of black slimy mold-like substance on interior of ice machine dripping onto cubes in bin(ice used for human consumption). Instructed person in charge to discard ice, clean and sanitize bin and maintain. Priority foundation #7-38-005. Citation issued.

- comments: observed damaged bottom on exterior door leading to second floor from basement. Instructed person in charge to repair and maintain.
