- comments: observed approximately 35 small rodent droppings on floor scattered along side and in corner of ovens, cooling units and shelving unit located in prep. Instructed to remove droppings, clean and sanitize said areas and eliminate pest activity. Priority foundation violation. #7-38-020 (a). No citation issued.

- comments: observed prep table in need of cleaning where microwave is stored. Instructed to remove dried food debris, clean and sanitize to prevent pest harborage.

- comments: observed floors throughout prep area as well as under all heavy equipment and storage shelving with heavy grease and food debris accumulated. Instructed to clean and maintain.

- comments: observed large hole in (orange) wall above prep table across from ovens, noted green wall in prep/storage with chipped paint/drywall, heavy grease debris buildup and surfaces not smooth and easily cleanable. Instructed to enclose hole and provide smooth and easily cleanable surfaces and maintain.
