- comments: paper towels dispenser install by the two compartment sinks on each kitchenette in each floors must be relocated at handwashing sinks or removed from said sinks.

- comments: no soap is provided at the handwashing sink at the kitchenette on each floor, where food is partially prepared and served. Must provide soap for all handwashing sinks for proper handwashing. Priority foundation violation:7-38-030(c), citation issued.

- comments: ice scoop must be stored in a clean and enclosed container between use.

- comments: 4-204. 18 out c 02/05/2020 condenser unit inside the walk-in freezer pipe is leaked, forming ice on the pipe. Instructed to repair.

- comments: the raw wood base under the two-compartment sink is in poor repair and not easily cleanable. Excess grime and rust on it. Repair/replace. The surface must be smooth, clean, and non-absorbent material.

- comments: we observed chlorine chemical sanitizing dish machines not properly dispensing chlorine sanitizer while dishes were being washed in different kitchenettes of each floor, 0 ppm chlorine noted after several attempts. Recommend using test strips to check sanitation levels daily. Tagged units' held for inspection' do not use. Priority foundation violation 7-38-025 citation issued.
