- comments: observed no person in charge with valid chicago food sanitation certificate. Instructed manager to designate person in charge during all hours of operation.

- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (chicken, beef, rice) must be on site at all times. (priority foundation 7-38-012)(citation issued)
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the only handwashing sink in kitchen with no soap. Instructed manager to ensure all hand washing sinks are supplied with soap. Manager did not have enough available hand soap on site. Manager sent employee to store to restock. Priority foundation violation 7-38-030(c) citation issued.

- comments: observed the only handwashing sink in kitchen with no paper towels. Manager sent employee to store to buy paper towels and restock. Priority foundation violation 7-38-030(c) citation combined with previous violation.

- comments: observed no sanitizer on site for sanitizing dishes. Instructed manager to ensure sanitizer is on site at all times. Manager sent employee to store. Priority violation 7-38-005 citation issued.

- comments: observed no certified food manager on site with food allergen training. Instructed manager to comply with requirement.
60. Previous core violation corrected
- comments: observed previous core violations not corrected within the specified timeframe. Report #2556308 dated 5-25-2022 instructed manager to repair baseboards outside women's toilet room and to ensure toilet room doors are self closing. Observed toilet room doors not self closing; observed missing baseboards along the wall outside women's restroom. Instructed manager to make corrections. Priority foundation violation 7-42-090 citation issued.
