Report date: Aug 21, 202438. Insects, rodents, & animals not present
- comments: observed pest activity with approx. 50 flies were noted during the inspection. Fly activity was noted throughout the facility on the main and 2nd floors. Observed approx 15 cocroches in the back cooking area under second from the left large cooking tank. Observed at least 10 flies in the container with masa. Instructed to consult pest control as additional services are needed to minimize or eliminate activity before re-inspection. Priority foundation violation #7-38-020(a). Citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: large food container next to the counter service area with open masa and at least 10 flies noted in it. Approx 100 lbs of masa worth $75. 00 was discarded and denatured. Must provide proper cover to prevent pest coming to the container. Priority violation 7-38-005. Citation issued.
54. Garbage & refuse properly disposed; facilities maintained
- comments: aprox 30 indoor refuse receptacles are overflowing with tortilla residues, masa. No covers, attracting flies and other rodents. Must provide tight fitting lids.
55. Physical facilities installed, maintained & clean
- comments: found ceiling located in rear cook area peeling over large open tank where food is prepared. Must repair and maintain to prevent physical contamination.

- comments: 2-102. 13 observed previous core violation from report #2568517 date 12/14/2022 not corrected: found no food handler training on site. Instructed manager to provide proof of food handler training for all employees. Priority foundation 7-42-090. Citation issued.
