- comments: 6-501. 111: observed over 60 small flies in facility. Observed small fly activity in rear of facility on first floor. Instructed to eliminate small fly activity in facility. - comments: 4-904. 13: observed exposed preset knife and fork on tables in dining areas. Instructed manager all preset tableware shall be protected from contamination by being wrapped, covered or inverted.
