- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (steak,chicken,sliced tomato,shreeded lettuce) must be on site at all times. Manager arrived on site at 11:40 a. M. (priority foundation 7-38-012)(citation issued)

- comments: observed improper temperatures of tcs foods such as 30lbs. Fully cooked italian beef-54. 3f in pot and 5 gallons bucket sour cream-50. 0f in 3- door reach in cooler. - comments: must label food storage containers(bulk,squeeze bottles and shakers)when food is not in original package.

- comments: observed a gap along bottom of double front entry door must make door tight fitting.

- comments: observed evidence of 1- live roach in crevices at service counter and several dead roaches at motors of both chest freezers and 2- door prep cooler. - comments: must clean grease and food debris build up from fryer cabinets and wheel bases,grill table,lid of grease trap of 3- comp sink.
