High Priority - Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee pulled up clothing after washing hands and putting gloves on and continued to prep customers orders.

High Priority - Rodent activity present as evidenced by rodent droppings found. Storage room separate from kitchen 1. Approx 12 rodent droppings on window sill in storage room separate from kitchen. 2. Approx 15 rodent droppings on bagel trays in storage room separate from kitchen. Operator cleaned, and sanitized area.

High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. 1. KITCHEN 2DOOR;Butter (54F); Cut Melon (57F) Tuna salad( 54F) 2. KITCHEN WALK IN COOLER RIGHT-; cooked noodles (50F); butter (50F) items held in unit less than 4 hours. Observed door in disrepair. Per operator foods not prepared or portioned today. Operator moved items to walk in meat cooler. **Corrective Action Taken**

High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Cookline; sausage (125F); home fries (122F), in unit less than 4 hrs, observed items under no temperature control. Operator reheated items. **Corrective Action Taken**
