- comments: observed personal beverages stored inside the milk cooler located in the common d & c area. Instructed manager to remove. Employees shall eat and drink only in designated areas where the contamination of exposed food cannot result. Manager removed the beverage during the inspection

- comments: observed milk crates being used as shelving throughout the walk-in freezer. Must remove and provide shelving 6' off the floor and provide floor accessibility to ensure adequate facility cleaning.
51. Plumbing installed; proper backflow devices
- comments: observed the handsink in the common c & d serving line to be stopped up and slow draining of water. Instructed to repair plumbing at the hands washing sink.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside dumpster with a missing lid located on the 3rd dumpster. Instructed to correct, replace and maintain to ensure tight fitting lids. Priority foundation violation #7-38-020(b). No citation issued.

- comments: observed burnt-out light bulbs inside the walk-in cooler and walk-in freezer. Instructed to replace and maintain.
