10. Adequate handwashing sinks properly supplied and accessible
- comments: 5-202. 12(a) observed no hot or cold running water in the pre-k toilet room in room 102. Instructed to restore adequate hot and cold running water to said hand washing sink in the pre-k room 102 toilet room. Priority violation 7-38-030(c).

- comments: 6-301. 14 observed no hand washing sign in the employee toilet room. Instructed to provide.

38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 50 rodent droppings in the storage closet located by the cafeteria exit door, about 30 rodent droppings located under the shelves in the prep area dry storage closet, about 10 rodent droppings under shelves located in the additional dry storage closet across from the main prep area, about 20 rodent droppings located behind the reach in coolers in the small cafeteria and about 5 rodent droppings located by the heater in the pre-k room 101. Instructed to remove rodent droppings, clean and sanitize affected areas and recommended contacting pest control for a service as well as placing rodent control devices in all areas mentioned (no rodent control devices noted in areas mentioned. ) priority foundation 7-38-020(a).

- comments: 6-202. 15 observed exit door not tight fitting or rodent proofed in the main cafeteria area, observed a one inch gap on the right side of the door along the wall. Instructed to make said door tight fitting and rodent proofed.

- comments: 4-601. 11(c) observed minor dust on the bottom prep table/hot holding unit shelves. Instructed to clean.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: 5-501. 17 observed no covered receptacle inside of the employee toilet room. Instructed to provide.

54. Garbage & refuse properly disposed; facilities maintained
- comments: 5-501. 15 observed overflowing and lids not tight fitting on outside garbage dumpsters. Instructed to never allow garbage to overflow and always keep all outside dumpster lids tight fitting at all times. Priority foundation 7-38-020(b).

55. Physical facilities installed, maintained & clean
- comments: 6-501. 114 observed clutter (accumulated crates) inside of the storage closet that is located in the main cafeteria. Instructed to remove unnecessary clutter and organize.

56. Adequate ventilation & lighting; designated areas used
- comments: 6-303. 11 observed burnt out/dull lighting inside of the dry storage closet located in the food prep area. Instructed to replace light bulb and always provide adequate lighting at all times.
