- comments: observed no valid city of chicago sanitation certificate on site at time of inspection. Instructed person in charge that certified manager must be on site with certificate at all times tcs foods are prepared or served (chicken, gyro). Priority foundation #7-2=38-012. Citation issued.

- comments: observed bulk ingredients in large white container not labeled(flour). Instructed person in charge to label all bulk ingredients not in original container.
38. Insects, rodents, & animals not present
- comments: observed approximately 80 mouse droppings scattered throughout rear storage area of facility in the following areas:behing large chest freezer, on the side of wall shelving, on shelving, on top of lids for dry ingredients, in between pallets where soda is stored. Instructed person in charge to remove all mouse droppings and sanitize all areas. Priority foundation #7-38-020(a). Citation issued.

- comments: observed food debris and build up inside of microwave located in rear storage. Instructed person in charge to clean and maintain.

- comments: obserrved debris on interior of 2 door cold hold unit in front prep area. Instructed person in charge to clean and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed mutliple water damaged ceiling tiles in storage area in rear. Instructed person in charge to replace and maintain.
