- comments: no written employee health policy on site. Instructed to provide policy with every employees verification. Priority foundation violation. 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: no vomit and diarrhea procedure and clean up kit on site. Instructed to provide procedure along with the necessary items as outlined in the policy. Priority foundation. 7-38-005.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed slight pink build up inside ice machine. Must clean interior of ice machine and maintain.

49. Non-food/food contact surfaces clean
- comments: observed accumulated food debris inside prep area 2-door reach in cooler. Must clean interior of cooler and maintain.

57. All food employees have food handler training
- comments: no food handlers training certificates on premises. Instructed manager all food handlers must have food handler certificates on premises.

60. Previous core violation corrected
- comments: the following previous core violations from report #2315892 dated on 10/15/19 not corrected-#43 observed ice scoop on top of machine improperly stored. Instructed to store in ice scoop holder,#51 observed no visible back flow device on ice machine in basement. Instructed to provide, #58 no allergen training certificates on premises. Instructed all city of chicago certified food managers must have food allergen training certificates on premises. All said previous core violations must be corrected by date of reinspection. Priority foundation 7-42-090.
