- comments: observed gaps around the side and bottom of the rear exterior door. Instructed person in charge to repair and maintain tight fitting at all times for the prevention of pest entry.

- comments: observed food items stored in the freezer uncovered. Instructed person in charge to cover all stored food items and maintain.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cardboard in use on the bottoms of tables and tops of equipment. Instructed the person in charge to remove and maintain all surfaces smooth and easily cleanable.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed waste water backup at three-compartment sink. When water is drained from the middle compartment, back up is observed in alternate compartments. Instructed person in charge to repair and maintain. Priority foundation #7-38-025. Citation issued.

- comments: observed no allergen training for pic with a manager certificate at the time of inspection. Instructed person in charge to obtain allergen training and maintain certificate on site.
