Report date: May 27, 20262. City of chicago food service sanitation certificate
- comments: observed no city of chicago food sanitation certificate posted on the premises. Management instructed that a certified food manager must be present with a valid city of chicago food sanitation certificate at all times while tcs foods are being prepared or served. Priority foundation violation 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand washing signs posted at the exposed hand washing sinks in the bar and food prep areas. Management instructed to provide at all hand washing sinks.
36. Thermometers provided & accurate
- comments: no visible thermometer observed inside of the refrigerator behind the front counter. Management instructed to provide visible thermometers inside of all prep coolers.

- comments: must label all dry storage containers with common name.
38. Insects, rodents, & animals not present
- comments: observed a rodent infestation throughout the premises. Observed approx. 1000 mouse droppings throughtout the food prep area, dishwashing area, bar area, behind the front counter, dining room, music and art classrooms, washrooms, basement and rear storage area. Observed droppings on the rolling carts in the food prep and dishwash areas, in the panels beneath the pizza oven, on the floor underneath the unused dishwashing machine, behind the 1-door freezer and rolling carts, under the beverage cooler and behind the bar coolers in the bar area, along the walls of the dining room and behind the music vending machine, in the dining room fireplaces, on the floor of the rear storage area, on the floor of the music and art classrooms, in the washrooms, and in the storage areas in the basement on the food prep tables where dry food is stored and on the floor throughout the basement storage area, in the motor housings of the unused and used coolers and freezers. Management instructed to clean and remove all rodent droppings throughout the premises. Additional pest control service is needed to eliminate the pest activity throughout. Priority violation 7-38-020(a). Citation issued.

- comments: observed a 1/2" gap along the bottom of the right side double exit door leading out to hamilton avenue. Management instructed to rodent proof all outer openings.

- comments: must invert or cover all clean equipment and utensils stored in the food prep area.
49. Non-food/food contact surfaces clean
- comments: clean the interior of the prep and bar coolers and freezers. Clean the rolling carts and stainless steel trays on the carts and prep tables. Clean the interior of the deep fryer and unused dishwashing machine. Clean all soiled utensils stored on the storage shelves.

- comments: remove the excessive clutter on the floor and corners of the basement leading to pest harborage conditions.

- comments: observed untreated sections of drywall above the pizza oven. Management instructed to paint or cover with a smooth and easily cleanable surface. Observed walls and ceilings of the prep and dishwash areas with chipped paint. Management instructed to scrape and repaint. Also observed gaps between the wall and floor in the dishwashing and food prep area. Management instructed to seal all openings to prevent pest harborage conditions. Regrout the floor tiles in front of the pizza oven where missing. Repair the open drywall section behind the door to the rear storage area.

- comments: observed no food handler training certificates provided. Management instructed that all food handling employees must show a valid food handler training certificate.
