- comments: no city of chicago food service manager on site as required while tcs foods being held at hot temperatures. Instructed that a city of chicago food service manager is required to be on premises while tcs foods are being heated, held and served. Priority foundation violation 7-38-012 issued.

- comments: no employee health policy on premises as required; must provide. Instructed the person in charge to provide documentation of employees responsibility to report any illness transmitted through food. Priority foundation violation 7-38-010 issued.

- comments: observed the rear side door not rodent proofed as required; instructed to rodent proof door to prevent possible rodent entry.

- comments: instructed to clean shelves in rear storage area, clean inside of three door cooler behind the front counter to remove standing water.

- comments: observed trash and other debris on ground around dumpster; instructed to clean and maintain area. Remove all trash from ground and make sure dumpster is not overflowing. Priority foundation violation 7-38-020(b) citation issued.

- comments: the floors need detail cleaning throughout the store under around and in between shelves and other equipment. Clean and maintain. Also clean floors in rear storage area.
