- comments: observed the previous core violations from report #2561624 dated: 8/2/22 not corrected; #53-observed no covered receptacles in the staff toilet rooms. Instructed to provide for all. #53 observed no self closing devices on the staff -oilet room doors. Instructed to provide as a means of preventing contamination of hands from touching doors after they have been washed. Instructed person in charge to come into compliance with violations and maintain.
