- comments: observed handwashing sink in the rear warewashing area covered by plastic wrap with drain plumbing disconnected and inaccessible by employees for handwashing. Instructed to remove plastic wrap and reconnect drain plumbing in order to allow for employee use and maintain at all times. Priority foundation violation. 7-38-030(c) citation issued.
36. Thermometers provided & accurate
- comments: observed no internal ambient air thermometer provided in 2-door reach-in cooler along main prep line. Instructed to provide at least one internal ambient air thermometer in each cold-holding unit used to store tcs food items and maintain at all times.
38. Insects, rodents, & animals not present
- comments: observed no pest control logbook maintained by staff on-site to maintain record of pest control company visits and operations. Instructed to provide updated pest control logbook with all required documentation as specified by law and maintain at all times. Priority foundation violation. 7-38-020(c) citation issued.

- comments: observed backflow prevention device at utility sink faucet with loose cap and in poor repair, but functional. Instructed to repair or replace backflow prevention device and maintain all plumbing fixtures in good repair.

Instructed to provide allergen awareness training to all certified managers and maintain at least one allergen awareness-trained certified manager with certificate(s) on-site at all times.
