- comments: observed no asterisk on menu stating that animal derived food, fried egg, is served raw, undercooked or cooked to order and observed no disclosure on menu stating animal derived food with an asterisk is served raw, undercooked or cooked to order. Corrected on site observed asterisk added to animal derived food, fried egg, that is served raw, undercooked or cooked to order. Observed disclosure stating that animal derived food, fried egg, with an asterisk is served raw, undercooked, or cooked to order added to menu before inspection completion. Priority foundation violation 7-38-005 no citation issued.
38. Insects, rodents, & animals not present
- comments: observed openings at base of front patio doors on eastside and southside of facility. Observed openings on left side of first floor rear emergency exit. Instructed to seal all outer openings to prevent pest entrance.

- comments: observed aluminium foil used as liner at bottom of prep tables in basement. Instructed to remove aluminum foil as liner, all non food contact surfaces shall be smooth for ease of cleanability.

- comments: observed black moldlike specks on upper area of ice shield and interior components. Observed orange moldlike substance around ice shoot. Instructed to clean and sanitize said areas of ice machine to prevent contamination of ice

- comments: observed two city of chicago certified foodservice managers with no proof of allergen awareness training. Instructed to provide proof of allergen awareness training for all employees with city of chicago certified foodservice manager certificates.
